1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Food safety certified person has a valid Servsafe but does not have a valid City of Philadelphia food establishment personnel food safety certification. Appliction was emailed.
6 No hot water was observed at the following handwash sink.
- Peak hot water at the handwash sink in the prep area was observed at 80.5 and 79.0 degrees F
- Peak hot water at the handwash sink in the bakery area was observed at 77.0 degrees F
- Peak hot water at the warewash sink the bakery was observed at 80.5 degrees F
- Peak hot water at the warewash sink was observed at 88.7 degrees F
- Peak hot water at the handwash sink at prep sink was observed at 82.3 degrees F
- Peak hot water at handwash sink at men's employee toilet room was observed at 81.5 degrees F
- Peak hot water at handwash sink at women's employee toilet room was observed at 80.0 degrees F
- Peak hot water at handwash sink at customer women's toilet room was observed at 82.9 degrees F
- Peak hot water at handwash sink at customer men's toilet room was observed at 77.8 degrees F
- Peak hot water at server's station handwash was observed at 76.5 degrees F
- Peak hot water at handwash sink in the bakery area at 81.2 degrees F
8 Paper towel dispenser empty at the handwash sink in the prep area. PIC provided paper towel at the time of the inspection.
8 Ice cubes and steel wool observed in the handwash sink in the prep area. PIC removed the items.
14 - The can opener had an accumulation of food residue on the food contact surfaces.
- Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
21 - Potentially hazardous ready to eat food, prepared in the food facility, located in the walk-in cooler, was not date marked.
- Opened commercially processed ready to eat food (cheese), located in the walk-in cooler, was not marked with the date it was opened.
36 Visible physical evidence of rodent / insect activity observed in the establishment.
- Small flies observed near the juice machine
- Mouse droppings observed on the floor near the toaster
- Mouse droppings observed on the floor perimeter of the bakery and warewash area
- Mouse droppings observed on the floor with the electrical boxes
- Mouse droppings observed on the floor near prep table in rear area
- Mouse droppings observed on the bottom of the prep table
- Mouse droppings observed on the floor near the drying rack
- Mouse droppings observed on the floor in the bakery area
45 - Inoperable cold holding unit observed in the prep area
- Inoperable reach-in refrigerator observed in the bakery area
- Missing compressor cover observed on the reach in refrigerator
- Inoperable walk-in cooler observed in the basement area
47 - Buildup observed on the gasket of the walk-in freezer
49 Leak observed in pipe at the handwash sink in the bakery area.
53 Low grout observed in the floor tiles in the prep area
53 - Grease accumulation observed on the ceiling tiles in the prep area.
- Water accumulation on the floor near the water heater.

1 Certified person did not have a Philadelphia certificate. Application given to the person in charge.
No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
8 Observations:
- Hot water was turned off at the handwash sink in the men's employee restroom. Hot water was restored.
- Paper towels were missing from several handwash sinks in the prep area, men's employee restroom, and warewashing area. Paper towels were provided.
- Handwash reminder sign was not posted at the handwash sink in the warewashing area. Sign was posted.
13 Raw eggs were stored above fruit in the cold prep refrigerator. Eggs were relocated.
21 Opened commercially processed ready to eat food (deli meats and cheeses), located in the walk-in cooler, and held more than 48 hours, was not marked
with the date it was opened. Dates were provided.
26 Working containers in waiter area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. Bottles were labeled.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible in a cold prep unit. Thermometer was provided.
41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients (breadcrumbs, panko).
45 The following observations were made :
- Defective reach-in refrigerator units in bakery area and prep area.
- Two compartment square cornered sink in the prep area
- Defective walk-in cooler in basement area.
45 Foil observed lining shelves in an under counter refrigerator in the prep area.
47 The following observations were made:
-Light food debris under equipment throughout front and back kitchen area.
- Food debris under racks in walk-in cooler in back kitchen and bakery area.
- Dust accumulation on fan vent cover in walk-in cooler in back kitchen area.
- Ice accumulation in basement walk-in freezer and bottom compartment of a cold prep unit.
* General cleaning is needed.
48 Hot water was turned off (under sink) at the handwash sink in the men's employee restroom. Hot water was restored.
51 Observations:
- Dust accumulation on the vent covers in the women's restrooms
- Defective urinal in the men's employee restroom
- Defective handwash sink in the men's customer restroom; hand sink was not clean to sight and touch
