2 Food safety certified person could not demonstrate knowledge of the foodborne illnesses deemed reportable, the symptoms that would require employee exclusion or report policy. Sample health policy emailed with the report.
8 -No handwashing reminder sign at any of the sinks in the bar area.
Handwashing reminder sign observed in sink under beverages on ice when beverages were removed from the sink.
Corrective action: Sign posted.
-The handwashing sink in the the bar area was filled with beverages on ice and not accessible at all times for employee use.
Corrective action: Beverages were removed from the handwashing sink. COS
-No paper towels were available at the handwashing sink in the bar area.
Corrective action: Handwashing sign posted.
Note: All handwashing sinks must be equipped with handwashing aids and handwashing reminder signs.
8 Handwashing sink in the food prep area does not have running water.
Corrective action: Water restored.
9 Food facility Person in Charge not able to provide documentation that pot of food carried into the establishment during the inspection is from an approved source.
13 -Handles of scoops observed in ice.
-Food prep sink was observed being used as the handwashing sink during the inspection. Wash, rinse and sanitize the food prep sink.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink in the bar was 0 to 100 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions.
14 Black stains and deep scratches observed on ice guard in the interior of the ice machine bin.
14 Per cook, dishes are washed and rinsed, but not sanitized.
Note: Same tablets used in the bar area may be used to sanitize dishes, utensils and and food equipment.
36 Visible physical evidence of rodent / insect activity observed in the bar area. Roach observed crawling on counter.
36 Front outer opening in the food facility does not protect against the entry of insects, rodents, and other animals. Gap observed beneath door in room with gambling machines.

-Debris and multiple dead flies observed on basement floor.
