13 - The ice scoop observed buried inside ice bin 2nd floor bar.
PIC took corrective action removed the ice scoop and dumped the ice during inspection.
-The quaternary ammonia concentration in the sanitizing solution of the 3 bay ware wash sink and one bucket in the 1st floor kitchen was greater than 500 ppm, rather than 200-400 ppm as stated on the manufacturer’s use direction.
*PIC initiated corrective action and lowered concentration to 200 ppm for the 3 compartment sink and one bucket from kitchen area.
14 The 3 bay sink was not properly set up for sanitation: washed, rinsed, and sanitized.
*Sanitarian instructed the person in charge how to set up the 3 bay sink.
. A return demonstration of Wash-Rinse-Sanitize setup was observed prior to completion of inspection.
14 The can opener had an accumulation of food residue on the food contact surfaces.
PIC took corrective action and washed, rinsed, and sanitized the can opener immediately.
45 -Domestic microwave "Solve" observed in the kitchen area.
-Broking ice bin lid observed at the 1st floor bar.
-Hand wash sink faucet 1st floor bar needs adjustment.
-No garbage grinder installed in the 2nd floor prep area (PR#19-0757).
-No dishwasher installed in the 2nd floor kitchen area (PR# 19-0757).
45 Raw wood shelves observed in the 1st storage area where contact surfaces are not smooth, easily cleanable or resistant to chipping and peeling.
53 -Gaps observed in the ceiling in the kitchen area.
-Big gaps observed on the wall at the electrical panel in the storage area and in the ceiling in the ware washing area.
53 Mop was observed in the bucket in the utility room 1st floor new kitchen.
53 Maintenance tools observed on the floor in the ware washing area.
54 - One ceiling light out 2nd floor prep area.

-A hand sink is not located within twenty-five feet of all areas of the first floor bar.
13 -Uncovered ice observed at the bar area and service station. Missing the ice covers.
-Several food items observed uncovered in the under-counter refrigerators, walk-in cooler, reach-in freezers, and reach-in refrigerators.
14 -Black residue observed on the interior of all ice machine bins (second floor/bar, shed, first floor/bar).
-Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
-The can opener had an accumulation of food residue on the food contact surfaces.
17 Cheeses sauce in the warmer is not reheated prior to holding in the warmer
*Potentially hazardous food that is cooked, cooled and reheated for hot holding shall be reheated so that all parts of the food reach a temperature of at least 74°C (165°F) for 15 seconds.

45 Thee following observations were noted:
-Missing a kick plate from the reach-in refrigerator in the kitchen area.
-An unused, defective cold prep refrigerator was observed in the food prep area.
-Domestic microwave "Solve" observed in the kitchen area.
-Glasswasher was dripping at second floor bar.
-The faucet at the hand wash sink in the ware washing area was dripping.
46 Food facility lacks quaternary ammonia sanitizer test strips to monitor sanitizer concentration.
49 -A mop sink was not observed.
-Backflow prevention devices are not installed on water supply lines of the ice machines.
53 -Damages floor surfaces were observed in the rear prep area and one walk-in cooler.
-Open pipe and utility line penetrations were observed in prep areas.
-Missing ceiling tiles were observed in the prep area near the hood.
54 Two lights out under the hoods.
54 Cover ventilations systems in all the restrooms observed with dust accumulation.
