8 Clutter, dishes and utensils observed in the hand wash sink in he food prep sink. The person in charge removed the items from the sink.
13 -Cassava leaf in the reach in refrigerator observed uncovered. -Rice in the front food prep area observed open and uncovered. The person in charge did not take corrective action during time of inspection, after speaking with Sanitarian about uncovered food items.
13 Rice scooper handle observed in direct contact with rice in the bag. The person in charge relocated the scooper to the correct position.
36 Front entry door opened in the food facility does not protect against the entry of insects, rodents, and other animals.
36 Visible physical evidence of rodent activity observed in various locations. Mouse feces observed on the front food preparation floor beneath shelves and under hot holding units, floor/ perimeter beneath the ware wash sink and handwashing sink in the kitchen, and in the front food service room floors. Live flies and fruit flies observed throughout the facility.
