36 Visible physical evidence of insect activity (dead water-bugs) observed on glue trap in the break room area.
**Dead or trapped birds, insects, rodents and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests.
**PIC took corrective action and discarded glue trap during time of inspection.
37 Food observed stored on floor in walk-in freezer and dry storage area and not stored at least 6 inches above the floor.
**PIC took corrective action and elevated food items during time of inspection.
45 The following were observed:
-Soda crates being used for elevation of food in walk-in cooler which are not smooth and easily cleanable
-Raw wooden palates being used for elevation of food in dry storage area which are not smooth and easily cleanable
45 Loose faucet handles observed at employee handwashing sink in the break room area.
47 The following were observed:
-Food debris around perimeters of floor in walk-in coolers/freezers
-Food debris on lower interior surfaces of reach-in refrigeration units/open display cases
-Food debris/residue on shelving units in food retail area
-Ice accumulation on ceiling and fan guards of walk-in freezers
51 The following were observed:
-Residue around faucet handles in men's and women's employee restroom areas
-Residue around interior surfaces of toilets in men's and women's employee restroom areas
-Static dust on air vents in men's and women's employee restroom areas
52 An accumulation of refuse and grease was observed around perimeters of near delivery entry door and behind refuse compactor.
**PIC was instructed to remove refuse from the premises during time of inspection.
**Refuse, recyclables and returnables shall be removed from the premises at a frequency that will minimize the development of objectionable odors and other conditions that attract or harbor insects and rodents.

53 Mop observed stored in a bucket of stagnant water in food retail area and not hanging in a manner that facilitates air drying.
