1 Employees (2) do not have a valid Food Establishment Personnel Food Safety Certificate issued by the Department. The application was processed on 5-17-24, but the certificate was not received.
8 Paper towel dispenser was empty at the hand wash sink in the sushi prep area. Paper towels were provided, while on site.
23 Sushi is served raw or undercooked to the customer's request; however, a written consumer advisory (on the menu, table tent, or placard) was not provided to the consumer. A consumer advisory was printed and posted, while on site.
27 Food facility did not following the variance procedure for sushi rice preparation, that was submitted and approved by the Department. A sushi rice log was not maintained for the day. The owner arrived to update the sushi rice log, while on site.
