1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. **The person in charge with the Philadelphia Food Safety Certificate arrived within 15-20 minutes of the inspection.
6 Warm water was not available at the handwashing sink in the men's restroom due to leak in valve. **Hands must be washed again at one of the kitchen area handwashing sinks prior to engaging in food prepping/serving activities.
8 Paper towel dispenser empty at the handwashing sink in the drink area. **Paper towels were supplied at the handwashing sink at the time of the inspection.
8 Shakers/utensils were observed in the handwashing sink at the drink area. **The items were removed from the handwashing sink at the time of the inspection.
13 - Raw animal foods (Chicken & Shelled eggs) were stored above ready to eat foods (sauces & beansprouts) in the walk-in cooler. - Observed uncovered beansprouts and soaking rice in the walk-in cooler. **The chicken and shelled eggs were relocated to the bottom shelf at the time of the inspection. **Beansprouts and soaking rice were covered with lids/plastic wrap at the time of the inspection.
14 - Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. - Observed improper set-up of the 3-bay warewashing sink. **Sanitarian displayed the proper set up for the 3-bay warewashing (Wash, Rinse, Sanitize).
35 Working bulk food ingredient storage containers (flour & Types of rice) are not labeled with the common name of the food.
41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients (flour, rice, etc.).
45 - Observed the shelf underneath the front of house counter/register made of raw wood which is not smooth and easily cleanable. - Observed foil and paper towels lining the shelves of the flat top grill.
46 Food facility lacks chlorine sanitizer test strips to monitor sanitizer concentration. of the dish machine.
46 Observed the 3-bay warewashing sink with a damaged stopper. **Employee purchased three new stoppers for the 3-bay warewashing sink at the time of the inspection.
46 Observed an accumulation of food debris in the catch basin of the 3-bay warewashing sink; food debris on equipment and utensils must be scraped over a waste disposal unit or garbage receptacle and/or may be removed in a warewashing machine with a prewash cycle.
47 Observed an accumulation of food debris in the prep-sink and 3-bay warewashing catch basin.
48 Observed the handwashing sink in the men's restroom lacking hot water due to a leak in the water valve.
49 Observed a steady leak of the hot water valve in the men's restroom.
50 Observed a basin underneath the handwashing sink counter to collect waste water of the ice machine behind the register area.
53 Observed an accumulation of food debris on the floor underneath the under-counter refrigerator and freezer, prep-table and steamer in the cooking area.
53 Observed missing/ajar ceiling tiles in the back warewashing are above the dish machine.
54 Observed a missing light bulb cover underneath the grill hood.
