20 Food(grits) in the reach in-refrigerator was held at 90 degrees F rather than 41 degrees F or below as required. PIC discarded the food item.

14 Dried food residue observed on the inside surfaces of the microwave oven. Food employee wash, rinse and sanitized the microwave oven during the inspection.

20 Eggs observed in the reach in refrigerator were held at 73.9 degrees F, rather than 45 degrees F or below as required. PIC discarded the eggs during the inspection.
26 Observed loose rodenticides on the floor and not contained in a covered, tamper-resistant bait station on the floor in the area with the ice machine.

14 The can opener had an accumulation of food residue on the food contact surfaces.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.

1 - Safety certified handler was not present at start of inspection, he arrived prior to completion of the inspection.
8 The following observations were noted: - Paper towel dispenser empty at the hand wash sink in the kitchen (near the dry storage area) area. - The hand wash sink near the dry storage area observed with food debris inside.
14 - The warewashing (3-compartiment)sink not properly setup for sanitation of utensils and equipment; dishes observed stored in rinsing bay of 3 compartment sink with papers and food debris. Corrective action: Sanitarian educated the PIC on how to properly setup the 3 compartment sink for sanitation of utensils and food equipment. A return demonstration of Wash-Rinse-Sanitize setup was observed prior to completion of inspection.
14 The following observations were noted: - Soda gun observed with a bug inside in the kitchen area. - The can opener had an accumulation of food residue on the food contact surfaces.
36 Rear door open during inspection, where outer opening is in the food facility does not protect against the entry of insects, rodents and other animals. PIC took corrective action and closed the door by the end of the inspection.
36 - Visible physical evidence of insect activity (fruit flies) observed in thought the facility area and mouse droppings observed on the floor inside the dry storage area. - A bug observed inside the soda gun in the kitchen area.
37 - Boxes with food observed stored on the floor in the walk-in freezer.

45 The following observations were noted: - Domestic microwave in the prep area. - Paper towel dispenser missing battery at the hand wash sink kitchen area. - The kick plate from the ice machine observed scotch taped in the dry storage area.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
46 - Food facility lacks sanitizer test strips to monitor sanitizer concentration.

14 - The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 50-100 ppm as required. - Low temperature ware-washing machine observed at 125 degrees F rather than 160 degrees F as required for sanitization purposes. **PIC was instructed to discontinue use of ware-washing machine and to utilize 3 bay ware-washing sink for wash, rinse, and sanitization purposes until serviced by maintenance.

8 The following were observed: -The handwashing sinks in the food prep area were blocked by a table and several cardboard boxes and not accessible at all times for employee use **PIC took corrective action and moved blockages from handwashing sinks during time of inspection. (COS) -Old food debris/residue observed in the handwashing sink in the rear food prep area
8 The following were observed: -Soap was not available at the handwashing sinks in the food prep area -Paper towel dispenser empty at the handwashing sink in the rear food prep area **PIC took corrective action and provided soap and paper towel at handwashing sinks during time of inspection.
11 One dented can observed on shelving unit in dry storage area. **PIC took corrective action and discarded can during time of inspection.
14 Low temped ware-washing machine observed at 138 degrees F rather than 160 degrees F as required for sanitization purposes. **PIC was instructed to discontinue use of ware-washing machine and to utilize 3 bay ware-washing sink for wash, rinse, and sanitization purposes until serviced by maintenance.
14 Black residue observed on the interior of the ice machine bin. **PIC was instructed to discard ice and properly wash, rinse, and sanitize interior surfaces of ice machine during time of inspection.
36 An insect control device located above shelving unit where clean food equipment is stored with potential to contaminate food, equipment, and / or utensils. **PIC took corrective action and moved shelving unit during time of inspection.
