26 Observed an unprotected package of rodenticide in the rear utility area. Rodent bait shall be contained in a covered, tamper-resistant bait station. Corrective action; PIC discarded the package of bait during the inspection.

41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients.

11 Observed food items stored in plastic grocery bags, rather than proper food grade coverings/containers, inside of the reach-in freezer, chest freezer, and reach-in refrigerator.
12 Observed shellstock in the walk-in cooler without tags/labels from where they were sourced from. PIC could not provide records or receipts of when the shellstock were received or their source. Sanitarian advised PIC to discontinue use of the shellstock during the inspection.
13 Mouse droppings observed on the interior surfaces of bowls used for food prep on the shelf next to the seasonings in the prep area. Food shall only contact surfaces of equipment and utensils that are properly cleaned and sanitized to protect from contamination.
13 - Observed several food items uncovered in the reach-in freezer, chest freezer, and in the lower compartment of the cold-holding prep unit. Sanitarian advised the PIC to cover these food items during the inspection.
14 Observed cooking utensils and equipment (pots/pans) soiled with food debris and grease. PIC informed sanitarian that equipment and utensils were left on stoves from the previous day of food prep. Food contact surfaces and utensils shall be cleaned throughout the day at least every 4 hours.
21 Potentially hazardous ready to eat food (fried rice), prepared in the food facility and held for more than 48 hours, located in the cold-holding prep unit, was not date marked. Sanitarian advised food employee to discard of the food items during the inspection, as they could not identify the date the RTE food items were prepared.
36 - Observed mouse droppings on the floor perimeters underneath the bulk dry storage in the rear hallway, underneath the shelves in the food prep and ware washing areas, next to and behind the refrigeration/freezer units in the cashier/prep areas, and in the walk-in cooler.
- Observed mouse droppings on the shelving surfaces in the front cashier area, underneath the rice cooker, and on the shelves storing seasonings and plates in the rear prep area.
- Observed rat droppings on the floor perimeters underneath the cooking equipment, next to the reach-in refrigerators, and underneath shelves in the rear storage area.
39 Moist wiping cloths observed lying on equipment and not stored in sanitizing solution.
47 - Observed a black mildew substance on the ceiling of the walk-in cooler.
- Observed dust accumulations on the fans inside of the walk-in cooler.
- Observed rust and stains on the storage racks in the ware washing area.
- Observed grimes and stains on the interior and exterior surfaces of the reach-in refrigerator in the kitchen/prep area.
- Observed mouse droppings, dust, and debris on the shelving surfaces in the front cashier area, underneath the rice cooker, and on the shelves storing seasonings and plates in the rear prep area.
