1 Certified person was not present at the start of inspection. They later arrived. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.

6 Food employees observed donning single use gloves without a prior hand wash. Employee was instructed to wash hands before donning gloves.

8 The handwash sink in the prep area and restroom was blocked by a bucket and not accessible at all times for employee use. Items were removed.

13 Raw animal food was stored above ready to eat foods (ginger, garlic cloves) in the walk-in cooler. Foods were rearranged.

14 Stoppers were not available at the warewashing sink. Stoppers were later provided.

14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours. Slicer was taken apart and cleaned.

45 -Raw wooden surfaces observed on the basement stairway and raw wood shelves in the retail and rear storage area. -Soda crates are used for elevation in the retail area and basement. -Cardboards are used as shelf liners in a reach in refrigerator and shelves in the retail area.

45 Observations: - Domestic chest freezers observed throughout facility. - Domestic microwave was observed at the cash register area. (Personal use) - Cracked floor tiles are observed in the walk in cooler.

47 Observations: - Dust accumulation was observed on vent covers in the walk in coolers. - Ice accumulation in the retail reach-in freezer - Residue observed on shelving in the retail refrigerators

51 Floor, wall, ceiling surfaces in the basement restroom are not smooth and easily cleanable. The handwash sink in the basement restroom is not sealed to the wall.

53 Cracked floor tiles observed in the retail area.

53 Food residue and debris observed under shelving and soda crates in the retail area.
