1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
* The person in charge with a FSC showed up during inspection.
8 The hand wash sink in the kitchen area was blocked by defective under counter refrigerator and not accessible at all times for employee use.
* The person in charge moved defective equipment.
45 - Defective under counter refrigerator observed in use for storage in kitchen area.
-Defective walk-in unit observed out back in trash area.
47 - Dust accumulation observed on fan covers in walk-in cooler.
- Heavy ice accumulation observed in chest freezer and reach-in freezer in basement.
-Debris observed on bottom surface of reach-in refrigerator in bar area.
49 Hand wash sink observed clogged and slow draining in downstairs prep area.

-Loose ceiling panel above reach-in unit in basement.

14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required at time of inspection.
-Orders: Discontinue usage of defective dishwasher immediately, utilize 3 compartment sink for all wash, rinse, and sanitizing procedures
26 Working containers in prep and bar areas, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical.

36 Mouse feces observed on side of staircase leading to basement and under rack shelving in basement, also along walls and corners.
