6 Observed food employee cutting ready-to-eat food (broccoli) using bare hands.
8 Observed the following:
- The handwash sink in the warewash area was blocked by buckets of oil and not accessible at all times for employee use.
-Soap was not available at the handwash sink in the warewash area.
Corrected on Site: Buckets were moved and soap was provided upon request.
13 Observed the following:
- Uncovered napa cabbage in under counter refrigerator
- Uncovered scallops in chest freezer
- Raw beef stored on top of chicken in walk-in refrigerator
- Raw beef and shrimp stored on top of ready-to-eat foods (green beans, rice, etc) in reach-in refrigerator.
36 Visible physical evidence of rodent activity (mouse feces) observed in the following areas:
- Underneath handwashing sink, prep sink, and three compartment sink
- On shelves used for storage of spices, flour, and utensils in food prep area
- In rear storage area underneath and on shelves used for storage of beverages, flour, sugar, etc.
- Underneath flat top grill and fryers
Observed dead roach in rear hallway.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
