11 Dented canned items observed in rear storage area. Corrective action: Food employee segregated the dented can from use.
20 Prosciutto was held at 46.2 degrees F rather than 41 degrees F or below as required. Corrective action: Food employee removed the prosciutto from the cold holding unit and put it in the walk-in cooler to cool down to proper temperature.

14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of a glass washer at the main bar was 0 ppm, and not 50-100 ppm as required. Corrective action: Facility agreed to stop using that glass washer until the sanitizer level is increased.
27 No pack or expiration date was written on meat that was sealed in the facility using reduced oxygen packaging. Corrective action: the meat was discarded.

26 Dichlorvos strips observed in the dry storage, food prep and bar areas were not being used according to the manufacturer's instructions. Dichlorvos strips are not to be used in food prep areas or in places where people are present for more than 4 hours a day. Corrective action: the dischlorvos strips were removed.
27 Food in the facility was vacuum sealed without the documentation detailed in the HAACP plan being completed. Corrective action: Vacuum sealed food was discarded.
