1 The person in charge observed with a serve safe certificate and not with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
* Sanitarian sent food safety certificate application link via email.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the kitchen area.
* The person in charge initiated corrective action by relocating chemicals to supply storage closet.
45 Closed display observed inoperable and used for muffin storage.
47 The following observations were made:
- Ice accumulation in chest freezers.
-Food debris on bottom surface of walk-in cooler under racks.
-Dust accumulation on fan vent covers in walk-in cooler.
-Food debris on bottom surface of reach-in freezer and under counter refrigerator.
51 - Raw wood ceiling border not smooth and easily cleanable observed in restroom.
- No lidded trashcan observed for proper disposal of sanitary napkins in restroom.
53 - Damaged concrete floor surface in rear storage/employee storage area.
- Water stained ceiling tiles observed in rear storage area.
53 Mop observed stored in mop bucket of water and not hung to allow for proper air drying.
