Inspection Date: 04/24/2025

6 The following observations were noted: -Food employees observed donning single use gloves without a prior hand wash. -Food employee did not follow proper handwash procedures. Food employee did not wash hands between changing tasks. **Corrective action was taken, person in charge explain and instructed food employees to properly wash their hands in between changing task and when changing gloves which they complied with and executed at time of inspection.**
13 Observed cooked food in a pot left uncovered to cool, stored on food prep table in unapproved basement food prep area. **Corrective action was taken, person in charge relocated cook food to storage shelf in walk-in cooler.**
20 Observed a bulk food container with uncooked cracked/scramble eggs and a carton tray of 24 eggs measured between 60 - 64 degrees F, rather than 45 degrees F or below as required in the food prep area. **Corrective action was taken, person in charge discarded the eggs at time of inspection.**
21 Observed potentially hazardous ready to eat food (i.e. packaged tuna salad), prepared by the food facility and held for more than 48 hours, located in the refrigerator in the dining area, was not date marked. **Corrective action was taken, person in charge placed date of when food were prepared at time of inspection.**
26 Chemicals were stored above or on the same shelf with food items, food contact/related articles in the main food prep area and basement storage rack and unapproved food prep area.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food throughout food prep and basement storage area.
35 Observed packaged ready to eat food, prepared by the food facility and stored inside refrigerator in the dining area for consumer "grab-n-go", is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
36 An unapproved insect control device (i.e. fly swatter) stored undercounter space in front prep/serve area with potential to contaminate food, equipment, and / or utensils. **Corrective action was taken, person in charge discarded the fly swatter at time of inspection.**
37 Observed evidence of food handling/prepation conducted in unapproved basement food prep area; during preparation, unpackaged food shall be protected from environmental sources of contamination, such as outside dust, construction or renovation debris, or ventilation dust.
41 Observed ice scooper stored in unlidded/uncovered container with debris inside on external top surface of ice machine bin in the basement storage area. **Proper Method of Storage in between use: (1) In a clean, dry location. (2) Where they are not exposed to splash, dust or other contamination. (3) At least 15 cm (6 inches) above the floor. (b) Additional storage requirements for equipment and utensils. Clean equipment and utensils shall be stored as specified in subsection (a) and shall be stored covered or inverted, and in a self-draining position that allows air drying.
42 Clean stacked dishware not observed stored inverted on storage rack above warewash sink. (1) In a clean, dry location. (2) Where they are not exposed to splash, dust or other contamination. 45 Observed cardboards used as floor liners in the basement storage area and walk-in cooler.
45 The following non NSF/ANSI equipment/appliances observed: 1. Waffle Maker 2. Microwave 3. Two Reach-in freezers 4. Under-counter mini refrigerator 5. Two Chest Freezers
46 Warewash sinks observed with residue in wash and rinse compartments and not clean to sight.
49 No backflow prevention device or hose bib installed at faucet of mop sink in the basement area.
61 Observed unapproved food prep area with a food prep table, dual compartment sink and ingredients stored in bulk containers used to make pancake batter mix in the basement storage area with exposed ceiling wood beams, no designated handwash sink observed. **Food establishment licensee and operation changes require submission of a Plan Review Application. 1 Food Safety Certified Person was not in the facility at the start of inspection. He arrived 20 minutes later. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
8 Soap was not available at the grinder handwashing sink in the food prep area. PIC provided soap during inspection.
21 Potentially hazardous ready to eat food (Tuna Salad, Crab Salad & Cut Melon), prepared in the food facility and held for more than 48 hours, located in the reach-in refrigerator, observed not date marked. PIC date marked food items during inspection.
36 Holes observed on the door screen in the food prep area where the outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
45 Missing panel observed on the exhaust ventilation hood. Panel was installed during inspection.
45 Non-NSF domestic chest freezer and small reach in refrigerator observed in the facility.
45 Cardboard lining observed on on floor surfaces in the basement and walk-in cooler.
47 The following observations were noted: -Ice accumulation observed inside the deep freezer in the basement. -Grease accumulation observed on wall backsplash at the grill. -Peeling plastic film observed on the side outer surfaces on the reach-in freezer in the food prep area.
53 The following observations were noted: -Porous ceiling tiles observed in the food prep area which are not smooth and easily cleanable. -Water stained ceiling tiles observed at the perimeters in the dining area, kitchen & bathroom.
53 Broom and dust pan observed on floor surfaces in the basement and not elevated or hung on a utility rack.
