11 Observed expired 4 half gallons of milk(1/2 a Gallon each 1/9/25)
The PIC removed and discarded the food items.
39 Moist wiping cloths observed lying on work surfaces and not stored in sanitizing solution.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
45 Observations of items not in good repair or not in use:
-Non NSF Approved Toaster
-Food warmer
-Meat cutter
-Meat slicer
-ice Maker
- walk in cooler etc.
-Damaged toilet seat in female toilet room.
-Leak beneath the 3 basin sink
47 Cleaning is needed inside the deli case, refrigerator, cold prep refrigerator, under counter refrigerator and walk in cooler where loose food particles were seen.
49 Repair toilet seat in the female toilet room.
51 A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
51 Observed toilet room fixtures and floor unclean to touch and sight.
53 -Damaged dry wall observed in one of restroom.
-Acoustic ceiling tiles observed in food prep area.
-Cracked floor observed throughout the facility.
-Old food debris observed on the floor beneath the cold prep refrigerator,
3 basin sink, every corners and walls.
53 A clutter of unused equipment observed in the rear storage and 2nd floor storage areas. Remove any items that are not necessary for the daily operation of the business.
*Cleaning and organizing needed in the entire facility.
54 Hood filters have an accumulation of dust and grease.
54 Observed one of the hood fluorescence light was inoperable.
The PIC was instructed to take immediate corrective action.
