1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Person in charge had a serv safe. Application was sent via email.
13 Raw chicken in the walk in cooler was observed not covered to prevent contamination. **Person in charge took corrective action.
18 Queso observed being thawed inside the ware wash sink on standing still water inside the sanitizer sink. **Person in charge removed from the sink and placed inside the food prep sink on running water.
45 The following was observed: -one out of service under counter refrigeration unit. -Damaged interior gaskets on a cold prep refrigeration units in food prep area
47 The following were observed: -light Food debris/residue around perimeters of floors in walk-in coolers/freezers -Food debris/residue on lower interior surfaces of under-the-counter/reach-in refrigeration units -Static dust accumulation on fan guards in walk-in coolers/freezers -Ice accumulation around perimeters of floors/ceilings of walk-in freezers

1 Philadelphia Certificate was not observed by Food Safety Certified person. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **An application for the Philadelphia Certificate was issued to PIC during time of inspection.
8 The following were observed: -Soap was not available at the handwashing sink in the women's employee restroom area -Paper towel dispenser empty at the handwashing sinks in the bar area and women's employee restroom area **PIC took corrective action and provided soap and paper towel at all handwashing sinks during time of inspection.
26 Chemicals were observed stored on the same shelf with food in cabinetry unit in bar area. **PIC took corrective action and moved chemicals to a chemical only designated area during time of inspection.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible in reach-in refrigeration units, under-the-counter refrigeration units, and cold prep refrigeration units in bar and food prep area.
41 Food dispensing utensil in the bulk ingredient container in walk-in cooler observed in direct contact with food and not having handle stored above the top of the food.
45 The following were observed: -One broken shelving unit in walk-in freezer -Handwashing sink in front food prep area is improperly mounted to wall -Damaged interior gaskets on some cold prep refrigeration units in food prep area -Non-operational hand dryers in women's employee restroom area

 Inspections
