Inspection Date: 03/31/2023

1 PIC observed without a Food Safety Certificate and City Cert. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Approved Food Safety Training Courses and an application for the City Certificate were issued during time of inspection.
8 The following were observed: -Soap was not available at the handwashing sink in the unisex employee restroom area -Paper towel dispenser empty at the handwashing sinks in the food prep area
14 The chlorine concentration in the sanitizing solution of the 3-bay mechanical ware-washing machine was 0 ppm, rather than 50-100 ppm as required.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay ware-washing sink was 0 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible in several refrigeration/freezer units/hot holding units.
36 Visible physical evidence of rodent activity (mouse feces) observed around perimeters of floors in food prep area/unisex employee restroom area, on exterior surfaces of shelving units/counter tops (under soda machine), and on interior surfaces of storage equipment where single use articles are stored.
45 The following were observed: -Non operational ice machine -Non-operational reach-in refrigeration unit -Missing cover plate on ice machine -Broken faucet handle at handwashing sink in unisex employee restroom area
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration for mechanical and manual ware-washing machines.
47 The following were observed: -Food debris/residue on lower interior surfaces of reach-in refrigeration/freezer units -Food debris/mouse feces on lower exterior surfaces of shelving units
48 Hot water is not available at handwashing sink in unisex employee restroom.
51 Lidded receptacle for disposal of feminine sanitary napkins was not observed in unisex employee restroom area.
51 The following were observed: -Static dust on air vent in unisex employee restroom area -Debris/mouse feces around perimeters of floors in unisex employee restroom area
52 Dumpster lids observed propped opened.
53 Thee following were observed: -Peeling paint on ceiling in unisex employee restroom area -Cracked tiles on wall surfaces in unisex employee restroom area
53 Debris and mouse feces observed around perimeters of floors throughout food prep area.
53 Mops observed stored in bucket of stagnated water and not hanging in a manner that facilitates air drying.
54 Hood filters have an accumulation of static dust.
54 The following were observed: -One light out under exhaust hood -One light out under exhaust hood where mechanical ware-washing machine is stored
