Hallway exit door on 1st floor is missing door sweep. 1st floor restroom wall vent is rusted with dust accumulation Ceiling tile missing above 2nd shower in 1st floor bathroom. Window sill paint observed chipping in 2nd floor women's restroom and in hallway. Residue observed along top of door frame. No lidded trash can observed. Men's restroom damaged ceiling, broken cabinet, broken toilet bowl lid, wall tile/vent missing. Water stained ceiling tiles in living room area & gap around sprinkler system. gap observed at floor board at windowsill. Room 1 observed with water stained ceiling and damaged wall surface behind bed. Room 2 water stained tiles. Room 3 Hole observed in wall. Med room is missing employee must wash hands sign. Room 9 Dust accumulation on blinds and ceiling fan. Cracked floor tiles in front of bathroom on 3rd floor. Room 10 observed under construction & unoccupied. Room 11 has damaged floor under shelf and window sill peeling Room 13 Cracked floor tiles and peeling paint on closet door. 3rd floor top of step covering is peeling and in need of repair.
