1 The Person in Charge failed to notify the Department of an imminent health hazard and cease operations.

8 Paper towel dispenser empty at the handwash sink in the food prep area.

13 Uncovered food observed in unts throughout facility.

14 The can opener had an accumulation of food residue on the food contact surfaces.
Black residue, pink slime observed on the interior of the ice machine bin around inner top lid.

21 Cooked and cut potentially hazardous ready to eat food(vegetable, cooked shrimps and pork), prepared in the food facility and held for more than 48 hours, located in the refrigerator, was not date marked.
Potentially hazardous opened commercially processed ready to eat food(fruit in syrup) located in the cold holding and refrigerator, and held more than 48 hours, was not marked with the date it was opened.
