1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. The person that was present at the beginning of the inspection has a class certificate but no Philadelphia Food Manager Certificate. A person with a valid Philadelphia Food Manager Certificate was not present.
45 Equipment does not meet minimum design standards: -Non-NSF or ANSI microwave, chest freezers, reach in freezer and small refrigerator/freezers observed. Note: there are commercial microwave and refrigerator present in the food prep area. -Domestic cabinets in the food prep area.
46 Food facility lacks QAC sanitizer test strips to monitor sanitizer concentration. The test strips observed are pH test strips.
51 Water-stained ceiling tiles in the staff and girl's toilet room on the basement level.
53 -Raw wood surface in the kitchen area in the 3 basin sink area. -Joints between the floor and wall surfaces are not sealed in the toilet rooms. -Gaps between surfaces observed in some classrooms.
53 Brooms and dust pans are stored on the floor rather than hanging on utility racks.
53 Observations: -General dust and debris on the floor perimeters in the basement hot water heater room. -Unused refrigerator and chest freezers are potential vermin harborage conditions.

8 -Paper towel dispenser empty at the handwash sink in the kitchen area. Paper towels were provided. -There was no can at the kitchen hand sink. A can was placed at the sink.
26 Observed insecticides and rodenticides not labeled by the manufacturer for food facility usage, in the classroom closet (Terro ant bait), utility closet (Home Defense spray) and hot water heater (poison blocks) areas. All pest control should be done by a licensed exterminator. The observed bottle of Home Defense insect spray and the Terro Ant Bait was removed during the inspection.
36 Visible physical evidence of rodent activity (rodent droppings) observed in the classroom closets and basement hot water heater areas.

This area was cleaned during the inspection. -Open poison blocks observed on the floor in the room near the hot water heater.
3270.77(a) -Wall and floor damage in the second floor Black classroom. -Openings in lower wall floor junction areas in a secocnd floor classroom closet, the basement classrooms and staff break room areas. -Peeling paint in the area leading to the hot water heater area.
3270.102(a) Large toys and stacked plastic crates are not anchored to prevent tipping in classrooms.
3270.107 Surfaces in the basement commercial refrigerator were soiled.
