1 City Certificate was not observed by Food Safety Certified person.
**No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**PIC was issued application for City Certificate during time of inspection.
8 The handwashing sink in the front service line area was blocked by a milk crate and not accessible at all times for employee use.
**PIC took corrective action and moved milk crate away from handwashing sink during time of inspection.
36 Dead insects observed on glue traps in dry storage room, front service counter, and food prep area.
**Dead or trapped birds, insects, rodents and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests.
**PIC took corrective action and discarded glue traps during time of inspection.
45 Two non-operable reach-in refrigeration units observed in the front and rear of the food prep area.

-Debris on lower interior surfaces of reach-in refrigeration units
-Debris on exterior surfaces of shelving units in front and rear food prep area
