13 Raw eggs were stored above other ready-to-eat foods in the walk-in cooler. *PIC relocated eggs at the time of inspection*
14 Black residue, pink slime observed on the interior of the ice machine bin in the basement prep area. *PIC drained ice and cleaned/ sanitized on site*
14 Soda gun holster observed with pink slime mildew at the bottom in the front main bar area. *PIC cleaned holsters on site*
22 Heavy cream and dressings were observed on the counter in the bar area- temping at 46 degrees. PIC could not provide a log for how long the potentially hazardous items were out; these items were relocated to a refrigeration unit.
33 Observed food items being thawed using unapproved methods: -Fish observed thawing at room temperature on prep rack in the prep area -Fish observed thawing in standing water in the prep sink
35 Multiple working bulk food ingredient storage containers are not labeled with the common name of the food.
36 Front entry door was propped open at the start of the inspection. This outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients. *Common bowls and food quart containers were removed at the time of inspection*
41 Observed utensils on the line being stored in a container of room temperature water. During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored in a container of water if the water is maintained at a temperature of at least (135°F)
43 Preset tableware observed at the dining tables. If tableware is preset, it shall be protected from contamination by being wrapped, covered or inverted.
45 Observed cardboard boxes being used to line the basement prep area. Surfaces of equipment that are nonfood-contact surfaces, but are exposed to splash, spillage or other food soiling or that require frequent cleaning, shall be constructed of a corrosion-resistant, nonabsorbent and smooth material.

20 Beverages and salad dressings were held at 51 degrees F rather than 41 degrees F or below as required in the under-counter reach-in refrigeration unit at the front small bar. **Person in Charge (PIC) lowered the temperature setting and brought the temperature below 41 degrees F during the inspection.
45 Cutting boards on cold prep units (salad prep units) in the kitchen observed with deep scratches and scoring, not allowing for effective cleaning and sanitizing.
45 -Broken/damaged/loose lid covers observed on the cold prep units (salad prep units) in the kitchen. -One of the "True" salad prep units lacks condensing unit cover. -Non NSF/ANSI sanitation certified equipment observed in the kitchen and basement: Whirlpool reach-in freezer and rice cookers (2). -Observed severely rusted and stained lower shelf surfaces of food prep tables in the basement kitchen. -Observed severely rusted and stained storage shelves inside multiple walk-in coolers in the basement. -Observed rusted and stained storage shelves in the basement dry storage areas.
45 Observed foil sheets lining the cheese melting machine's holding racks, which are not easily cleanable.
47 -Dusty fan guards observed in multiple under-counter refrigerators at the front bar, front small bar and the rear room bar. -Dusty fan guards observed in the salad prep units(3) in the kitchen. -Dusty fan guards observed inside multiple walk-in coolers. -Black mildew stains observed on the ceiling surface of the walk-in cooler containing beer kegs in the basement. -Dust observed on the interior ceiling surfaces of multiple walk-in coolers in the basement. -Food debris, dust and greasy substances observed on the interior floor surfaces of all walk-in coolers and the walk-in freezer in the basement.
47 Pink residues observed on the interior frame of the ice machine bin (not in direct contact with ice). **PIC removed all ice and cleaned the ice machine bin frame during the inspection.
51 Dusty ventilation cover observed in the employee's restroom.
53 -Stained wall and ceiling surfaces observed throughout the facility. -Greasy substances observed on the wall and ceiling surfaces throughout the bar/kitchen areas and the basement. -Food debris, dust and greasy substances observed on the floor throughout the facility, concentrated under and behind the food preparation and cooking equipment in the kitchen, behind bar counters, under and behind the storage shelves in the basement, and under the shelves in the liquor room. -Dead roaches observed on the floor near the walk-in cooler containing beer kegs in the basement.
53 -Observed damaged/broken floor tiles in the kitchen. -Observed damaged/broken ceiling tiles at the basement 2-bay sink area. -Missing ventilation cover in mop sink room.
54 Observed 1 reach-in freezer's light bulb is burnt out (the one containing beef brisket and bread).

8 -Soap was not available at the hand sink in the rear room bar. -Soap was not available at the hand sink in the small bar at the front. **PIC provided soap for hand sinks during the inspection.
14 Black residues observed on the interior surfaces of the ice machine bins (2) located in the basement kitchen, in direct contact with ice.
21 -No date marking on ready to eat food in the cold holding units: wrapped cheese, mac & cheese and wrapped pork.
36 Restricted Hotshot fly killer strip observed at the front bar area with potential to contaminate food, equipment, and / or utensils.
36 -Mouse droppings observed behind the rear room bar counter and under the storage shelves along the floor perimeter of the basement. -Fruit flies observed in the basement area, concentrated around the trash cans and indirect drains.
37 Beverage bottles observed not stored at least 6 inches above the floor in the liquor room. **PIC moved all beverage bottles onto the shelves.
42 Drainboards for glassware are adjacent to bar handwashing sinks without adequate splash protection (at the rear room bar and the small bar at the front).
43 Single-use utensil boxes were not stored at least 6 inches above the floor in the basement. **PIC moved all single-use utensil boxes onto the storage shelves during the inspection.
45 -Non NSF/ANSI sanitation certified equipment observed in the kitchen and basement: Cuisinart slow cooker, Whirlpool reach-in freezer, rice cookers (2). -Severely rusted and stained lower shelf surfaces of food prep tables observed in the basement kitchen. -Severely rusted and stained shelves observed inside multiple walk-in coolers in the basement. -Broken/damaged lid covers observed on the cold prep units (salad prep units) in the kitchen.

47 Cooking equipment, including the smoker, ovens, and ranges/stoves in the kitchen, observed to have an accumulation of static grease on their exterior surfaces.

54 -Two light covers were observed damaged/cracked above the ware washing area in the basement. -One light is burnt out under the hood.

63 Person in Charge of premises has not posted the appropriate warning signs regarding the dangers of consuming alcoholic beverages during pregnancy. **PIC provided warning signs during the inspection.

27 Food facility is not following the HACCP plan for ROP of raw and cook/chill foods that was submitted and approved by the Department. *Inconsistent labeling procedure observed (raw ROP sauces labeled with "use by" date while ROP smoked meats/poultry were date labeled with preparation date). *PIC initiated corrective action by changing labels on smoked meats/poultry to used by date. * ROP raw food packages lack proper wording (ROP raw food labels must include instruction to maintain the food at 41 degrees or below and discard the food within 30 calendar days of its packaging)
