14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 50 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. Freshly prepared sanitizer solution was still only 50 ppm. Third attempt yielded correct concentration.

4 An employee's open beverage container was observed in a food preparation area.
Corrective action: Lid was placed on beverage during the inspection.
6 Insufficient amount of cold water at handwashing sink made it impossible to wash hands. Temperature of the water reached 150 degrees F with cold water on. Food employees instructed to wash hands at the garbage grinder sink until cold water is restored.
Warm water must be available at handwashing sinks for proper handwashing.
8 Handwashing sink in the toilet room observed soiled.
8 Soap was not available at the handwash sink in the toilet room.
Instructed the food employee to provide soap. Upon returning to the store for exit interview, no soap was available in the toilet room.

Food employees instructed to move items 50 F and below to a colder case.
Upon Sanitarian's return to go over the inspection report, all food that was thrown away or moved to colder refrigerators was observed back in the dysfunctional open display case.
