- Defective walk-in cooler observed in basement.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
46 Food facility lacks correct sanitizer test strips (Quat) to monitor sanitizer concentration.
47 - Wiping cloth's observed in use as liner in under counter refrigeration unit.
- Dust accumulation on shelves in Dj area.
-Dust accumulation observed on fan vent cover in basement walk-in cooler.
-Dust accumulation along hot hold equipment.
51 - Floor board observed loose in 1st floor restroom.
- Damaged floor tiles in 2nd floor restroom.
- Sink in need of caulking in 2nd restroom.
-Faucet observed loose in 2nd floor restroom.
53 The following observations were made:
- Food debris on floor surface under prep table in kitchen area.
-Grease accumulation and dust being hot hold unit on floor surface.
-Damaged wall on 2nd floor.
-Hole observed in floor on 2nd floor.
-Wall paint observed peeling on 2nd floor.
-Bottle caps under hand wash sink in 2nd floor bar.
-Damaged concrete floor surface in basement.
53 The following observations were made:
-Missing ceiling tiles on 2nd floor.
- Miscellaneous clutter observed in the basement.
- Accumulation of dust and debris observed on floor perimeters throughout the basement.

* The person in charge relocated trashcan.
26 Chemicals( sanitize tablets) were stored above or on the same shelf with food, equipment, and/or single service articles in the service/ware wash area.
* Sanitize tablets were removed and table was cleaned.

- Miscellaneous clutter on side of defective under counter refrigeration unit in upstairs bar area.
- Unnecessary clutter observed in the basement.
- Accumulation of dust and debris observed on floor perimeters throughout the basement.

* The inspector reviewed food borne illnesses with the person in charge.
8 Paper towel was not available at the hand wash sink in the kitchen and bar area.
*Paper towel was provided.
8 No sign or poster posted at the hand wash sink in the restroom areas to remind food employees to wash their hands.
* Hand wash sign was provided.
8 Hot water in restrooms and bar areas observed at 140-142 degrees F and not at 100-110 degrees F as required. - Water is too hot to properly wash hands.
14 Black residue, pink slime observed on the interior of the ice machine bin around interior lids in basement area.
