*Person in charge cleaned can opener and corrected on site.
21 Potentially hazardous ready-to-eat food prepared in the food facility (Chicken and Sauces) and stored in the refrigeration units was not date marked.
Note: The person in charge date marked the appropriate food.
37 Case of oil stored on direct floor surfaces, is not stored at least 6 inches above the floor.
39 Moist wiping cloths observed lying on kitchen surface area and not stored in sanitizing solution.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.

-Kick plate missing for the reach-in freezer prep area.
-Domestic reach-in freezer observed in the food prep area.
47 The following observations were noted:
-Ice accumulation observed in the chest freezer in the basement area.
-Food debris observed inside the chest-freezers from prep area.
53 -Porous ceiling tiles observed in the food prep area that is not easily cleanable.

1 - The food safety certified handler was not present at start of inspection, he arrived prior to completion of the inspection.
1 The person in charge did not demonstrate adequate knowledge of the Food Code as evidenced by incorrect responses to food safety questions. Certified Food Manager must train the person in charge.
8 - The handwash sink in the cooking area does not have single use towels, continuous towels, or air drying device.
- Soap was not available at the handwash sink in the cooking area.
Person in charge provided soap and paper towels during inspection.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was more than 400 ppm. PIC emptied the sink then refiled it with sanitizer measured at 300 ppm.
14 - The can opener had an accumulation of food residue on the food contact surfaces.
- Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
*Sanitarian advised to washed, rinsed, and sanitized the slicer and can opener.
20 Food (cheese and dressing) in the under counter refrigerator held at 55 degrees F and in the cold prep unit (cheese, pizza sauce, sliced tomatoes) was held at 58 degrees F rather than 41 degrees F or below as required.
The person in charge took corrective action and discarded the cheese, dressing, sliced tomatoes, and pizza sauce during inspection.
21 No date marked for cooked food, cheese and deli meats in reach-in refrigerators, under-counter refrigerators, cold prep units, and walk-in cooler observed during inspection.
34 - Food temperature measuring devices for ensuring proper food temperatures are not calibrated and/or functioning properly in the under-counter refrigerator cooking area.
- Missing probe thermometer.
36 -Gaps observed underneath the basement back door where outer opening in the food facility does not protect against the entry of insects and rodents.

37 Food are not stored at least 6 inches above the floor in the basement area.

PIC provided soap during inspection.
19 Pizza was held between 82 degrees F and 90 degrees F rather than 135 degrees F or above as required.
PIC removed the pizza from serving area and placed in the oven.
21 Several ready to eat food items store in the reach-in refrigerators, under-counter refrigerators, and walk-in cooler observed without dates.
Sanitarian advice the PIC to provided dates for all the food was prepared in the reach-in refrigerators, under-counter refrigerators, and walk-in cooler.
36 House flies observed in the facility (prep area, basement area and restroom area).
