47 Equipment cleaning: **Grease has accumulated on surfaces inside the mechanical exhaust hood. Please clean.
49 No prep sink observed. The food code requires facilities to have a separate sink, with an open air drain, for the sole purpose of washing food items. If installing a food prep sink is not possible or practical you can apply for a variance. A variance application will be provided.
53 Cleaning is needed in the area around the deep fryer and chest freezer due to grease accumulation.

1 The Person in Charge did not demonstrate adequate knowledge of how to set up three-basin sink. Sanitizer was prepared in the wash basin.
6 Food employees observed donning single use gloves without a prior hand wash.
13 Observed the following: -Uncovered food (cheese, tomato sauce, beef patties) in walk-in refrigerator. -Raw chicken stored next to deli meat and meatballs -Observed fly landing on uncovered cooked bacon in food prep area.
20 Food in the bain marie refrigerator was held at 40- 50 degrees F rather than 41 degrees F or below as required.
21 Observed multiple foods (deli meat, cheeses, beef patties, etc), prepared in the food facility and held for more than 48 hours, located in the walk-in cooler, was not date marked.
36 Observed fly swatters in food prep area.
36 Visible physical evidence of rodent (mice) and insect (flies) activity observed in the food prep and POS area.
45 The garbage disposal unit is not working properly (electrical switch is malfunctioning)
