45 - Cardboard lining observed in reach-in cooler. - Crates observed being used inside walk-in cooler.
45 Observed the following: - Seating area booths observed with torn seating surfaces for customers observed in dining area. Replacements are needed. - Defective cooking equipment observed: Under counter reach-in cooler in waitstaff area (used as dry storage), Reach-in cooler.

4 Observed an employee's open beverage container in the food prep area
6 Observed the following: -Food employees observed donning single use gloves without a prior hand wash. -Observed employee handling ready-to-eat foods (sausage) without prior handwash or gloves
8 Observed the following: -Lack of soap in both men's and women's restroom -The handwash sink in the kitchen area was blocked by a chair and not accessible at all times for employee use. -No hot water available at handwashing sink in women's restroom **Women's restroom was labeled 'Out of Order' until it can be fixed and chair was moved upon request.
13 Observed the following: -Uncovered sausages and potatoes inside of reach-in refrigerators -Raw fish stored above and next to ready-to-eat foods in reach in refrigerators -Uncovered muffins and sausage inside of reach-in freezer -Raw beef stored above to and next to ready-to-eat foods in reach-in freezer. -Uncovered sauces inside of walk-in cooler
26 Observed chemical bottle in rear storage area without a label.
36 Observed employee's bringing in unapproved animals (cat or dog) into facility.
37 Beverages are not stored at least 6 inches above the floor.
41 Serving utensils located in waitstaff area and dish washing area stored with food contact surfaces exposed to hand contact when retrieved for service.
45 Observed the following: - Seating area booths observed with torn seating surfaces for customers observed in dining area. Replacements are needed. - Defective cooking equipment observed: Under counter reach-in cooler in waitstaff area (used as dry storage), Reach-in cooler, Ice machine and exhaust hood in back food prep area. - Pipe connected to dish washer observed with drip. Water is being dripped into metal pan on floor. -Bowl used for scooping sitting on top of ice inside of ice machine.
47 -Surfaces inside the walk-in cooler are in need of cleaning (fan guards, shelves). Grill surface and fryer needs to be cleaned. Old dirt, food residue and oil observed on surfaces. -Observed ice accumulations on floor, doors, and ceilings of walk-in freezer
48 Hot water delay observed at hand wash sink in waitstaff area.
53 Cleaning is needed in the kitchen, particularly the wall and table near the grill, under fryer and grill surfaces and floor behind all cooking equipment. Dirt, and old food residue observed.
54 Observed the following: -Employee's personal vape and earphones on prep table -Employee's personal shoes stored on top of flour in rear storage area. Corrected on Site: Person in Charge removed items upon request.
