47 Black residue observed on walls, ceiling and shelving units in walk-in refrigerator. Cleaning required.
51 Small stain observed on ceiling tile in the toilet room inside of the food prep area.
53 -Wall repair around plumbing between warewashing sinks and handwashing sink in the food preparation area is not smooth, non-absorbent and easy to clean.(Bare wood) -Broken concrete observed at floor perimeters in some of the warehouse areas.
53 General floor cleaning is needed to remove dust and debris from floor perimeter behind rack near closet with sink.
54 Lighting intensity above the warewashing sink is 76 to 110 lux rather than 220 lux. Lighting intensity above the slushie machine is 336 to 400 rather than 540 lux.

8 Handwashing sink also used as mop sink. A handwashing sink may not be used for other purposes. Corrective action: Per owner, the handwashing sink is needed in that area, so it will be cleaned and sanitized and used only as a handwashing sink. A mop sink to be installed nearby.
14 Facility does not have sanitizer for the slushie machine. Corrective action: Sanitizer obtained.
45 Bare wood observed on the wall behind the slushie machine and legs of the countertop holding the slushie machine. Note: Surfaces should be smooth, non-absorbent, and easy to clean.
46 Slushie pitcher could not be placed in middle basin of warewashing sinks due to faucet placement. Per owner, faucet to be raised so that pitcher fits into all compartments. *Must be completed before slushie machine use can be approved.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration. Please purchase quaternary ammonia test strips. (Please DO NOT purchase pH test strips.)
47 -Reach-in freezer has an accumulation of ice. -Black residue observed on exterior of reach-in freezer and behind door of reach-in freezer.
