47 The following observations were made:
- Dust accumulation on fan vent covers in walk-in cooler.
-Food debris on floor surface of walk-in cooler under storage rack.
-Food debris on bottom surface of under counter refrigeration unit in down stairs prep area.

45 -Leak observed at faucet of rear main 3 compartment sink.

1 * The Certified Person did not provide a Food Safety Certification from the Health Dept. The Certified Person did provide a copy of the money order and a copy of the application during time of inspection. The certification from the Health Dept. will need to be obtained.
14 * The main ware wash unit did not reach the required 180 degree F for proper sanitization of food equipment's and food contact surfaces. The Person in Charge did make the proper repair and 180 degrees F & over was observed during time of inspection.
14 * Interior surface of th 3-basin sink is not clean to sight & touch. The Sanitarian advised the Person in Charge to wash, rinse and sanitizer the interior surface of the 3-basin sink during time of inspection.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food in basement, where needed.
36 * Mice droppings are observed under and/or behind food equipment's and shelves in the kitchen area and 1st floor bar area. The establishment is advised to obtain a Pest Management Service.
41 * Utensils and knives are stored in stagnant water in the kitchen area.
45 * Condensation leak is observed in the pizza prep unit in the kitchen area.
47 * Mouse droppings are observed on the lower shelves of the food preparation tables in the kitchen. The Person in Charge did clean the shelves during time of inspection.
* Interior and exterior of the reach-in freezer and bain marie units are in the of cleaning.
49 * No backflow devise was observed at the ice machine.
* Proper indirect drain line with air gap was not provided for the soda-gun holders in the 2nd floor bar area. (Corrected on Site)
* Missing floor drain cover near the ice machine and in the 2nd floor bar area, where it is needed.
51 * Covered receptacle for sanitary napkins was not observed in the women's restroom.
52 * Uncovered trash receptacle are observed in the bar area and the kitchen prep area.

* Wall paneling in the kitchen area is not flush to the wall, where it is needed.
* Ceiling and walls in the bar area and in the kitchen area is in need of cleaning, where it is needed.

14 Due to mouse activity in basement on food equipment including prep table and slicer unit, all food contact surfaces mentioned must be cleaned and sanitized prior to future usage
