13 Multiple food items were uncovered in the chest freezers and refrigerator. *PIC corrected on site.
21 Multiple RTE food items, prepared in the food facility and held for more than 48 hours, located in the walk-in cooler and the front refrigerator, were not date marked. *PIC corrected on site.
36 Front entry door observed propped open at the start of the inspection- this outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
37 Food is not stored at least 6 inches above the floor: cooking oil observed being stored directly on floor in the prep area and items stored directly on the floor in the walk-in cooler
41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients(seasonings, flour and rice), and inside the rice cooker.

**Person in Charge promptly provided covers for the food items.
14 The quaternary ammonium (QAC) concentration in the sanitizing solution of the 3-bay ware washing sink was 100 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions.
**Person in Charge added more tablets, and the concentration of the sanitizing solution tested at 200 ppm after corrective action was taken.
36 Visible physical evidence of rodent / insect activity:
Mouse feces observed on the floor perimeters of the kitchen and storage rooms, concentrated under multiple food preparation and cooking equipment in the kitchen, at the stair entrance to the basement, and along the floor/wall junctures in the small dry storage room and rear storage room.
41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients (seasonings, flour and rice).
45 The surfaces of the wood shelving units in the walk-in cooler are not smooth and easily cleanable.
