1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Sanitarian emailed a copy of the Philadelphia Food Safety Certificate Application to the person in charge.
8 Paper towel dispenser empty at the handwashing sink in the warewash/prep area. **Paper towels were supplied at the time of the inspection.
14 Observed 'wash' and 'sanitize' basins not properly holding water solution due to stoppers. **Dish machine will be used until new stoppers have been supplied.
21 Opened commercially processed ready to eat foods (Half wheel of parm, block cheese and prosciutto), located in the walk-in cooler, and held more than 48 hours, was not marked with the date it was opened. **Dates were placed on foods at the time of the inspection.
41 - Food container observed without handles in direct contact with bulk food ingredient (Pizza Salt). - Food dispensing utensil in the flour observed in direct contact with food and not having handle stored above the top of the food. **Utensils were removed from the ingredients at the time of the inspection.
49 No backflow prevention device observed on the mop sink and the water line of the ice machine.
49 - Observed a leak in the pipe under the wash basin of the 3-bay warewash sink. - Observed the handwashing sink in the diaper changing station restroom slow draining.
53 Observed mop head not properly hung to dry to prevent insect harborage.
53 Observed brooms and dustpans not properly elevated in the utility closet.
54 Excessive heat & steam from high-temp dishwasher observed, without mechanical ventilation of sufficient capacity.

8 Observed water at the handwashing sinks not properly reaching 100 degrees F. **Person in charge was able to get the handwashing sinks to the proper temperature at the time of the inspection.
14 The food-contact surfaces of equipment and utensils did not reach 165 degrees F during the sanitizing cycle of the high temperature dishwasher as evidenced by the failure of the temperature indicator. **Person in charge was able to correctly set-up the 3-bay sink with the proper sanitizing concentration.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
48 Observed water not reaching a temperature of 100 degrees F. **Hot water was restored at the time of the inspection.
49 Observed slow draining handwashing sink in the Diaper changing restroom.
51 Observed an accumulation of static dust on the vent in the gender neutral restroom.
53 Observed opening from a missing vent above the door in the lower storage area.
54 Observed low lux lighting intensity in the walk-in cooler in the lower area.
