8 The following observations were noted in the toilet room:
-Paper towel dispenser empty at the handwash sink.
-Soap was not available at the handwash sink.
**Corrective action was taken, person in charge provided paper towel and soap at handwash sink located in the toilet room at time of inspection.**
26 Observed Chemicals spray cans/bottles stored above food and equipment in the rear storage room.
**Corrective action was taken, person in charge relocated the chemical spray containers to bottle storage cart at time of inspection.**
37 Observed cased of prepackage beverages stored on direct floor surface, not stored at least 6 inches above the floor in the rear storage room.
45 The following observations were noted:
-Unused NSF/ANSI approved existing exhaust hood, a handwash sink and 2 compartment sink with garbage grinder installed.
-Unused Non NSF equipment, electrical 2 burner hotplate, toaster, coffee marker, refrigerator and microwave stored in the unused prep and enclosed back outdoor areas.
-Non NSF storage racks in the unused prep area and retail areas.
-Non NSF/ANSI chest freezer in the retail area.
46 The following observations were noted:
-Warewash sink observed with food residue and coffee pitcher in compartments and not being cleaned and sanitized prior to use.
-No cleaning tools(i.e. sponges) available to clean the internal perimeter to the warewash sink compartments and/or the unclean dishware.
**Corrective action was taken, person in charge obtained the necessary cleaning tools then cleaned and sanitized the warewash sink prior to set-up at time of inspection.**
51 Observed no lidded trash bin in the toilet room.
51 The following observations were noted:
-Toilet bowl and toilet seat not clean to sight.
-Overflow of trash in the unlidded trash bin.
-Trash on floor surface.
53 The following observations were noted:
-Porous ceiling tiles.
-Water stain ceiling tile in the rear kitchen area.
-Unsealed gaps between wall surface and base coving in the rear storage room.
-Hole in ceiling covered by a cardboard, cardboard is not smooth and easily cleanable in the toilet room.
-Raw wood bean installed behind the toilet bowl, not smooth and easily clean in the toilet room.
-Peeling base coving in the rear kitchen area.
-Damage wall surface behind the toilet bowl in the toilet room.

-Non NSF/ANSI chest freezer in the retail area.
47 Observed two dead roaches and food debris in the dual compartment sink in the rear kitchen area.
53 The following observations were noted:
-Porous ceiling tiles.
-Water stain ceiling tile in the rear kitchen area.
-Unsealed gaps between wall surface and base coving in the rear storage room.
-Peeling base coving in the rear kitchen area.
-Damage and cracked wall surface of basement staircase and behind basement entrance door.
53 Observed 2 dead roaches and food debris in the dual compartment sink in the rear kitchen area.
53 Idle broom and dust pan stored on direct floor surface in the rear storage room rather than on utility rack.

**Corrective action was taken, person in charge discarded the fly swatters at time of inspection.**
