45 - Repair the door in the bathroom Pre-K 4-5 classroom. - Missing ceiling ventilation cover in the infant classroom.
53 The following observations were noted: - Water ceiling stains observed in bathroom second floor classrooms. - Missing ceiling cover ventilation in the infant room. - Seal the gaps and the base coving in the 3rd floor classrooms. The classrooms are not in use during inspection.
54 - Dust accumulation observed on the cover vents at second floor classrooms.

45 Non-NSF/ANSI refrigerators and microwaves observed in the classrooms.
49 A leak was observed under the handwash sink (one restroom) in the 2nd floor classroom. Person in charge put in a work order for the sink to be service.
51 Dust accumulation observed on the vent covers in the restrooms. Baseboard needs to be resealed in the restroom of the 2nd floor classroom.
53 Unsealed (unpainted) wall surfaces/missing base coving observed in the stairway to 3rd floor classroom area.
53 Idle broom/dustpan was not elevated in the pre-K class. Maintenance tools were properly elevated.
53 General cleaning needed in the 2nd/3rd floor classrooms due to residue observed.
54 Dim light observed in the 2nd floor class restroom.

3270.21 Cubicles in some classrooms (afterschool, kindergarten) were not mounted to the floor/wall.
3270.66(b) Spray bottle was observed not labeled in one of the classrooms. Bottle was labeled.
3270.75(c) Gloves were missing from the first aid kit in the pre-K classroom. Missing item was provided.
3270.76 Baseboard needs to be resealed in the 2nd floor class restroom. General cleaning needed in the 2nd/3rd floor classrooms due to residue observed.
