26 Loose rodent bait observed on the floor in the closet with the refrigerator.
Note: Rodent bait must be contained in a covered, tamper-resistant bait station.
36 Visible physical evidence of rodent activity (mouse feces) observed on the floors in the the classroom closets on the 3rd floor.
37 Refrigerator observed in a closet on the 3rd floor. Loose rodent bait observed on the the floor around the refrigerator.
43 Ensure that all food utensils/containers are stored at least 6" above the floor.
45 -Handle on reach-in refrigerator door in kitchen is in disrepair.
-Reach-in freezer in kitchen is malfunctioning. Ambient temperature of the refrigerator was 76 degrees F at the time of the inspection.
-One sink in the men's toilet room is inoperable.
-(Ref 46.501) The following equipment is not NSF/ANSI approved: 2 basin drop-in sink, porcelain handwashing sink, refrigerator/freezer combo and electric pan on 3rd floor.
45 Microwaves in the kitchen and throughout the building are not NSF/ANSI approved.
53 Some floor tiles are missing at the handwashing sink in the 3rd floor CERC room.
58 Alteration or construction began prior to plan submission and approval. A completed Plan Review Application with proper plans, drawn to scale, showing all design features, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the, Office of Food Protection.
