1 Food Safety Certified person at the start of inspection. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Food Safety Certified person arrived by end of inspection.
1 3 bay ware-washing sink in main bar area observed improperly set up with two sanitizer sinks. **PIC took corrective action and properly set up the sink with wash, rinse, and sanitize sections.
8 No sign or poster posted at the handwashing sink in the main bar area to remind food employees to wash their hands. **PIC took corrective action and provided handwashing sign during time of inspection.
8 Old food residue observed in the handwashing sink in front bar area. **PIC took corrective action and discarded food residue and properly wash, rinsed, and sanitized interior surfaces of handwashing sink during time of inspection.
21 Potentially hazardous ready to eat food (bread pudding), prepared in the food facility and held for more than 48 hours, located in the refrigerator, was not date marked. Date was provided.
26 Unapproved fly trap observed in the side bar area. Trap was discarded.
26 Chemical bottles located in food prep area and main bar were not observed labeled. **PIC took corrective action and labeled chemical bottles during time of inspection.
33 Food observed thawing at room temperature in the prep area. Food was relocated to the walk-in cooler.
37 Food is not stored at least 6 inches above the floor in the walk-in freezer.
42 Clean food equipment being stored wet, and not in a manner to allow air drying... wet nesting.

47 The following were observed: -Dust accumulation observed on fixtures (ceiling fans) above the bar -Grease accumulation on interior surfaces cooking equipment (fryers) -Ice accumulation on interior surfaces of cold prep unit
49 Slow draining handwashing sink observed in main bar area.
53 Food debris, static dust/debris, and dead insects observed around perimeters of floors in food prep area/ main and rear bar.
53 Stained ceiling tiles observed throughout facility.
54 Hood filters have an accumulation of static dust and grease.

41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients (flour, bread crumbs).
45 Cutting boards were observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
48 Hot water delay was observed at the handwash sink in the front bar area. Note: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food facility.
53 Static dust observed on the ceiling tiles in the food prep area.

6 Food employee observed handling food with bare hand contact before following proper handwashing procedures. **PIC was instructed to discard potentially hazardous food and to follow proper handwashing procedures prior to handling food during time of inspection.
8 The following were observed: -Paper towel dispenser empty at the handwashing sink in the rear bar area -Soap was not available at the handwashing sink in the front bar area -Hot water does not reach peak demand at front and rear bar. Hot water at a temperature between 70-74 degrees F. **PIC took corrective action and had handwashing sinks serviced. Hot water reached peak demand and was observed at a temperature of 100-110 degrees F by end of inspection. (COS)
11 One dented can observed on shelving unit in food prep area. **PIC took corrective action and discarded can during time of inspection.
13 Uncovered food items observed in under-the-counter refrigeration units in food prep area. **PIC took corrective action and provided covers for all uncovered food items during time of inspection.
14 The following were observed: -2/3 sink stoppers needed for 3 bay ware-washing sink in food prep area -2/3 sink stoppers needed for 3 bay ware-washing sinks in front and rear bar areas -Chemical sanitizer needed for rear bar area
14 Low temp mechanical ware-washing machine observed 90 degrees rather than 120 degrees F as required by manufacturer for chemical sanitizer final rinse cycle. **PIC took corrective action and had mechanical ware-washing machine serviced by maintenance during time of inspection.
36 Visible physical evidence of insect activity (flies) observed in front bar and rear bar areas, dining area, and employee restroom areas.
43 Single-use articles observed improperly inverted in food prep area.
45 The following was observed: -Unused equipment stored outside near outdoor dry storage area -One faucet on 3 bay ware-washing sink non-operational -Cold prep refrigeration unit is non-operational -Condensation leak in under-the-counter refrigeration unit in food prep area
45 Food contact surface of several cutting boards are not smooth, easily cleanable and resistant to pitting, cracking, or scratching.
47 The following were observed: -Food debris/residue on lower interior surfaces of reach-in refrigeration units, under-the-counter refrigeration units, and closed display cases in food prep area and front/rear bar -Food debris around perimeters of floor in walk-in cooler/freezer -Ice accumulation along perimeters of floor/ceiling in walk-in freezer -Ice accumulation on interior surfaces of reach-in freezer unit/cold prep units in food prep area -Static dust on fan guards in walk-in cooler/freezer and reach-in refrigeration units/freezers in food prep area -Food debris on shelving units in food prep area -Grease accumulation around food equipment on cooking line and around perimeters of floor in food prep area -Pooled water on lower interior surfaces of under-the-counter refrigeration unit in food prep area
51 The following were observed: -Static dust/rusted air vent in men's and women's employee restroom areas -Loose faucet handles at handwashing sinks in men's employee restroom area

11 Potentially hazardous foods were held at a temperature between 48-51 degrees F in cold prep refrigeration unit and under-the-counter refrigeration unit in food prep area rather than 41 degrees F or below as required. **PIC was instructed to discard all potentially hazardous foods and to utilize other refrigeration units until serviced during time of inspection.
13 Uncovered food items observed in walk-in cooler and under-the-counter refrigeration units in food prep area. **PIC took corrective action and provided covers for all uncovered food items during time of inspection. (COS)
14 The chlorine concentration in the sanitizing solution of the 3-bay ware-washing sink was greater than 200 ppm, rather than 50-100 ppm as required. **PIC took corrective action and diluted chlorine concentration to 50-100 ppm during time of inspection.
20 Potentially hazardous foods were held at a temperature between 48-51 degrees F in cold prep refrigeration unit and under-the-counter refrigeration unit in food prep area rather than 41 degrees F or below as required. **PIC was instructed to discard all potentially hazardous foods and to utilize other refrigeration units until serviced during time of inspection.
26 Working containers in rear bar area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. **PIC took corrective action and labeled chemical bottles during time of inspection.
36 The following were observed: -Visible physical evidence of rodent activity (mouse feces) around perimeters of floor in the outdoor dry storage area and food prep area -Visible physical evidence of insect activity (flies) under prep sinks in front food prep area **PIC took corrective action and properly discarded mouse feces during time of inspection.
45 The following was observed: -Damaged interior gasket on walk-in freezer unit in food prep area -Unused equipment stored outside near outdoor dry storage area
45 Food contact surface of several cutting boards are not smooth, easily cleanable and resistant to pitting, cracking, or scratching. **PIC was instructed to replace all cutting boards during time of inspection.
47 The following were observed: -Food debris/residue on lower interior surfaces of reach-in refrigeration units, under-the-counter refrigeration units, and closed display cases in food prep area and main bar area -Food debris around perimeters of floor in walk-in cooler/freezer -Ice accumulation along perimeters of floor/ceiling in walk-in freezer -Ice accumulation on interior surfaces of reach-in freezer unit/cold prep units in food prep area -Static dust on fan guards in walk-in cooler/freezer and reach-in refrigeration units/freezers in food prep area -Food debris on shelving units in food prep area -Grease accumulation around food equipment on cooking line and around perimeters of floor in food prep area -Pooled water on lower interior surfaces of under-the-counter refrigeration unit in food prep area
