**Bowls were removed from the sink at the time of the inspection.

- Observed the 4th handwashing sink in the day room restroom running.
- Observed the pipe on the 4th handwashing sink leaking.

PM-305.1 Observed broken glass in the shower area in Guest Restroom #1 containing the handicap stall.

8 - Paper towel dispensers empty at the handwashing sinks in the upper living area restrooms.
- Soap was not available at the handwashing sink in the handicap stall of Guest Restroom #1.
- Soap was not available at the handwashing sink on the right hand side in the Guest Restroom containing multiple showers.
11 Severely dented canned item observed in dry goods area.
**The canned item was removed from the shelf at the time of the inspection.
14 Observed the 3-bay warewash sink setup incorrectly (Sanitize & Wash) instead of Wash, Rinse and Sanitize.
**The 3-bay warewash sink was setup correctly at the time of the inspection.
20 Food observed in the reach-in cooler on the dock area was held at 51.4 degrees F rather than 41 degrees F or below as required.
**The reach-in cooler was placed out of order at time of the inspection.
34 Temperature measuring device (internal thermometer) for ensuring proper temperatures of equipment was not available or readily accessible in reach-in refrigerator.
**A thermometer was placed in the refrigerator at the time of the inspection.
36 Visible physical evidence of rodent activity (mouse droppings) observed in the dry storage area near the walk-in freezer and ice machine.
41 Common bowl/food container was observed without a handle in direct contact with bulk food ingredient (brown sugar).
41 Food dispensing utensil in the sugar observed in direct contact with food and not having handle stored above the top of the food.
45 - Observed broken soap dispensers in Guest Restroom #2.
46 Food facility lacks chlorine sanitizer test strips to monitor sanitizer concentration.
46 Observed utensils and equipment not being properly rinsed so that abrasives are removed and cleaning chemicals are removed or diluted through the use of water due to a broken stopper.
**The 3-bay warewash sink was reset up to accommodate for rinsing.
49 Observed clogged utility sink in the upper floor janitor closet.
51 Observed an accumulation of static dust on the vents in all the restrooms.
53 Observed mouse dropping under the racks/ around the floor perimeter of the the small dry storage closet near the walk-in freezer and ice machine.
54 Observed blown out lights in Guest Restroom #2.

**Person in charge removed the cup from the flour at the time of the inspection.
