1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Corrected On Site: An email with an application for the Philadelphia Food Safety Certificate was sent.
8 Soap was not available at the handwash sink.
45 Observed cardboard linings on cabinet shelves underneath three basin sink.
47 Observed debris on floor inside walk-in freezer.
53 Floor cleaning is needed under the 3 basin sink and behind refrigerators.

A City of Philadelphia Food safety certificate was emailed with this report.
4 An open employee's beverage container was observed in a food preparation area. Beverage was removed during the inspection.
41 Utensils for scooping ice cream observed in cold water dip well.
41 Food dispensing utensil in the ice chest observed in direct contact with iced and not having handle stored above the top of the ice.
45 - Kick plate observed hanging off reach-in freezer.
47 - Cone display need to be cleaned. Heavy accumulation of crumbs observed. - Ice accumulation observed inside chest freezers. - Dust accumulation observed on ceiling vents.
53 Broom and dust pan observed on floor and not elevated when not in use.
53 Walls in utility closet are stained. Cleaning is needed.

14 Black residue, pink slime observed on the interior of the ice machine bin. Note: PIC discontinued use of machine and began cleaning during time of inspection.
37 Items stored in the walk-in freezer, on the stairway, and in the upstairs storage are are not stored at least 6 inches above the floor.
45 Crates observed used for elevation are not approved. Refrigeration unit on the right side the ware washing sink is non operational.
45 Cardboard observed lining shelves in the cabinets are not smooth and easily cleanable.
47 Floors in the walk-in cooler observed in need of cleaning. Shelf around ice cream machine observed in need of cleaning.
49 Ice accumulation observed on floor of walk-in freezer.
51 Toilet room door is not self-closing.
53 Floors observed in need of cleaning in the storage area. Old mice feces observed on HVAC system.
54 Dust observed in vent inside the walk-in freezer.
