21 Observed opened commercially processed ready to eat food, located in the cold prep refrigerator, and held more than 48 hours, was not marked with the date it was opened.
36 Visible physical evidence of rodent (mice) activity observed in the rear storage area.

45 Domestic non-NSF ANSI blender, and chest freezer observed in the storage.
47 Observed ice accumulation inside of chest freezer in retail area.

- Cleaning needed behind chest freezer due to mouse droppings.
- Grease accumulation on hood exhaust.

- Underneath fryer and flat top grill due to food debris
