1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **PIC has only Servesafe Certificate (8/30/26). Food Safety Certificate Application form was emailed during this inspection.
21 Potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the refrigerator, was not date marked. PIC date marked the food items
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
39 Moist wiping cloths observed lying on kitchen table and not stored in sanitizing solution.
41 Dispensing utensils shall be stored with their handles above the top of the food (chick peas, rice, flour, sugar, etc) within the containers.
45 -Inoperable garbage grinder observed in the rear food prep area. -Ware washing faucet observed not reachable(short) to the sanitizing Basin.
47 Clean and sanitize all kitchen equipment, handwashing sink, garbage grinder and basement storage area.
48 Slow drainage observed on the hand washing sink in the kitchen area.
50 Observed buckets of waste water stored in the basement storage. PIC was instructed to take immediate action.
51 The employee restroom door is not self-closing.
53 -Floor behind the inoperable cooking equipment observed with debris.
53 Clean and sanitize all floors throughout the facility. Especially kitchen, ware washing, toilet room and basement storage area.
53 Maintenance tools such as brooms, dust pan, mops, vacuum cleaners and similar items shall be stored so that they do not contaminate food, equipment and utensils.
54 Hood filter has an accumulation of static dust and grease.
54 Observed hood system has no lighting bulb.

13 Raw chicken was stored above raw beef and lamb in the reach in refrigerator. The person in charge relocated the raw chicken to the lower shelf during the time of inspection.
45 Inoperable hood, stove, pizza oven, and mixer observed in the rear food prep area.
45 Card board lining bottom shelf on the prep table in the rear is not smooth and easily cleanable.
51 The employee restroom door is not self-closing.
53 -Floor behind the inoperable cooking equipment observed with debris.
54 Rear hood filter has an accumulation of static dust.
