36 The following were observed: -Visible physical evidence of rodent activity (rat and mouse feces) observed around perimeters of floors in the warehouse area/employee restroom areas, and on lower exterior surfaces of raw wooden palates -Potential rodent harborage areas inside the food facility observed in the warehouse area due to accumulation of unused equipment -Potential rodent harborage areas on the exterior of the building perimeter observed in dumpster area due to unsealed lids on dumpster and dumpster lids remaining propped opened
36 Outer openings observed on lower surfaces of delivery entry doors in the food facility which do not protect against the entry of insects, rodents, and other animals.
37 Food is not stored at least 6 inches above the floor in the warehouse area.
45 The following were observed: -Raw wood being used to line shelving units, which are not smooth and easily cleanable -Raw wooden palates being used for elevation, which are not smooth and easily cleanable -Cardboard being used to line shelving units and raw wooden palates, which are not smooth and easily cleanable
45 Potential rodent harborage areas inside the food facility observed in the warehouse area due to accumulation of unused equipment.
47 Rat and mouse feces observed on lower exterior surfaces of raw wooden palates.
51 The following were observed: -Static dust on air vents in both men's and women's employee restroom areas -Rat feces around perimeters of floors in both men's and women's employee restroom areas
52 Potential rodent harborage areas on the exterior of the building perimeter observed in dumpster area due to unsealed lids on dumpster and dumpster lids remaining propped opened.
53 The following were observed: -Unfinished wall surfaces -Holes on lower perimeters of wall surfaces -Base coving needed around perimeters of floors in warehouse area
