8 Paper towel dispenser empty at the handwashing sink in the restroom area near the room where the mop sink is located. **Paper towels were supplied at the handwashing sink at the time of the inspection.
36 Outer opening in the food facility does not protect against the entry of insects, rodents, and other animals: observed a gap underneath the garage door on Phillip St.
45 Observed some of the elevation racks/stands made of non corrosion-resistant, absorbent and non-smooth material being used to store the bulk alcoholic beverages in the walk-in cooler.
51 Observed dust accumulation on the vent in the warehouse office restroom.
53 Observed wood chips/dust and packaging debris on the floor in the walk-in cooler.
53 - Observed water damaged porous ceiling tiles at the front customer retail area. - Observed a missing ceiling tiles above the adult gaming machine on the right side of the customer retail floor.

45 Equipment is not NSF or ANSI-Sanitation approved: cardboard floor liner in walk-in cooler & raw wood shelving area in walk-in cooler; a few plastic soda crates used for beverage case elevation.
50 The water condensation from the walk-in cooler drained to the sewer line inside so inspector could not verify proper indirect waste drainage.
