36 Gaps observed around the bottom of the drive-thru window not protecting against the entry of vermin.
45 Plastic crates used to store products in the walk in refrigerator.
53 Observations: -Rust observed on the wall covering over the 3 basin sink.
54 Employees' bag stored on a shelf with boxes of products rather than in the employee lockers. The bag was moved.
67 Printed nutrition information was not available for customers. The person in charge printed out this information during the inspection. He stated that he would teach staff to provide this information when requested by customers.

14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was less than 150 ppm as stated on the manufacturer's use directions. The bottle was almost empty. A fresh bottle was installed at the 3 basin sink. The sanitizer solution was tested at 200 ppm after the new bottle was installed.
26 Observed insecticides (fly spray) not labeled by the manufacturer for food facility usage, in the office area. The person in charge stated that she would discard the spray. All pest control should be done by a licensed contractor.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
36 Visible physical evidence of insect (large and small flies) activity observed in the service / prep, warewash, storage and dining areas.
