1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
2 Food safety certified person could not demonstrate adequate knowledge 0freportable foodborne illnesses.
8 Old food residue, dishes and utensils observed in the handwash sink.
Corrective action: Items were removed from the sink.
13 -The food prep sink is soiled and has some build-up in the bottom corners of the sink.
-Cell phone observed on the cold prep refrigerator.
14 Dishes were observed in the handwashing sink and were then moved to the food prep sink. Warewashing sinks were heavily soiled and had rust on them.
14 The quarternary ammonia concentration in the sanitizing solution from the sanitizer dispenser was 0 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions.
Concentration corrected.
19 Potentially hazardous food was held at 120 degrees F rather than 135 degrees F or above as required.
Corrective action: Items were place back in the oven to be heated to 165 degrees F while the hot-holding units were prepared for use.
20 Potentially hazardous food was held at 45 to 50 degrees F rather than 41 degrees F or below as required. Per owner, food from shopping was recently put in refrigerator and the refrigerator was open during prep. Instructed the person in charge to turn the unit down and to keep it closed. Unit will be checked again prior to exit interview.
Corrective action: Temperature of the refrigerator decreased by the end of the inspection.
