3270.76 Gaps observed: - in the ceiling in the kitchen - in the ceiling and behind the hand wash sink, toddler room 2nd floor Observed missing floor tiles in the afterschool classroom on the 2nd floor. Damaged wall unisex restroom 2nd floor. Hole observed on the floor and on the wall (near heater) in the kitchen area. Holes around the AC ducts in the toddler classroom.

1 The food safety certified handler was not present at the start of inspection, he arrived prior to completion of inspection. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
41 Plastic cup observed without handle in direct contact with bulk food ingredients (cereals).
45 Observed a Non NSF approved refrigerator/freezer in the Infant Room. Domestic microwave "Sentinel" observed in the kitchen area.
47 Pool of water observed inside the reach-in refrigerator.
48 The hand wash sink hot and cold water in one of the unisex restroom observed with low pressure.
53 Mop observed in the bucket in the kitchen area.

She arrived after that.
