1 Food Safety Certified person observed without City Certificate. If city certificate is not obtained within 30 days upon next inspection, violation grants for closure. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **An application for the City Certificate was issued during time of inspection.
6 The following were observed: -Portable handwashing sinks in Early Toddlers Classroom, Bumblebees Classroom, and Pre-School Classroom observed non-operational -Hot water does not reach peak demand at handwashing sink in Infant Classroom. Hot water observed at a temperature of 80 degrees F and below
8 The following were observed: -Soap was not available at the handwashing sink in the food prep area -Hot water does not reach peak demand at handwashing sink in Infant Classroom. Hot water observed at a temperature of 80 degrees F and below **PIC took corrective action and provided soap at handwashing sink in food prep area during time of inspection. (COS)
8 Handwashing sink in Infant Classroom was not clean to site and touch. Black mold observed on interior surfaces of handwashing sink. **PIC was instructed to wash, rinse, and sanitize interior surfaces of handwashing sink during time of inspection.
11 The following were observed: -Four dented cans on shelving unit in food prep area -Milk located in reach-in refrigeration unit in Infant Classroom with "Sell By" date 8/13/22 **PIC was instructed to discard dented cans and milk during time of inspection.
14 Handwashing sink in Infant Classroom was not clean to site and touch. Black mold observed on interior surfaces of handwashing sink. **PIC was instructed to wash, rinse, and sanitize interior surfaces of handwashing sink during time of inspection.
36 Visible physical evidence of rodent activity (mouse feces) observed on lower interior surfaces of handwashing sink's cabinetry unit in the food prep area and around perimeters of floor under shelving unit in food prep area.
36 Outer openings observed around lower interior surfaces of rear door in the food facility, which does not protect against the entry of insects, rodents, and other animals.
