41 The following were observed: -Food dispensing utensil in the bulk ingredient containers/ice machine observed in direct contact with food and not having handle stored above the top of the food -Common bowls or food containers observed without handles in direct contact with bulk food ingredients
45 Cardboard observed being used to line shelving units in food prep area/walk-in cooler which is not smooth and easily cleanable.
45 Food contact surface of several cutting boards are not smooth, easily cleanable and resistant to pitting, cracking, or scratching.
47 The following were observed: -Food debris on lower interior surfaces of reach-in refrigeration units and under-the-counter refrigeration units -Food debris around perimeters of floor in walk-in cooler/freezer -Grease accumulation on cooking equipment in food prep area -Food debris/residue on shelving units in food prep area -Food debris/residue on interior surfaces of microwave unit in food prep area -Ice accumulation on fan guards in walk-in freezer -Static dust on fan guards in walk-in cooler
49 Slow draining handwashing sinks observed in food prep area.
51 Air vents observed in both men's and women's employee restroom areas.
53 Food debris/residue and static dust accumulation/debris observed around perimeters of floor in dry storage area, food prep area, and front service counter.
54 Hood filters have an accumulation of static dust and grease in food prep area.

8 Old food debris/residue observed in the handwashing sinks in rear of food prep area. **A handwashing facility may not be used for purposes other than handwashing.
8 The following were observed: -Paper towel dispenser empty at handwashing sink in front service counter area **PIC took corrective action and provided paper towel at handwashing sink during time of inspection. (COS) -No sign or poster posted at the handwashing sinks in both the men's and women's employee restroom areas to remind food employees to wash their hands **A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees
13 The following were observed: -Uncovered potentially hazardous food in walk-in freezer -Raw chicken stored above sauces in the walk-in cooler
14 Potentially hazardous food observed stored on drying rack for clean food equipment of three bay ware-washing sink and not on food prep table. **PIC took corrective action and moved potentially hazardous food and properly washed, rinsed, and sanitized exterior surfaces of drying rack during time of inspection.
21 Potentially hazardous food ready to eat food, prepared in the food facility and held for more than 48 hours, located in the walk-in freezer and under-the-counter-refrigeration units were not date marked.
26 Chemicals were stored on the same shelf with food in the food prep area and front service counter area. **PIC was instructed to move chemicals to a chemical only designated area during time of inspection.
26 Working containers in food prep area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. **PIC was instructed to label all chemicals during time of inspection.
36 Visible physical evidence of rodent / insect activity (mouse feces, dead water bugs) observed behind behind ice machine in front service counter area.
37 Food is not stored at least 6 inches above the floor in walk-in freezer.

13 Used food equipment observed being stored in food prep sink. **PIC was instructed to only store used food equipment in mechanical or 3 bay ware-washing stations and to properly wash, rinse, and sanitize interior surfaces of food prep sink during time of inspection.
14 Pink slime observed on the interior of the ice machine bin. **PIC was instructed to discard ice and properly wash, rinse, and sanitize interior surfaces of ice bin during time of inspection.
21 Potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the under-the-counter refrigeration units in food prep area, were not date marked. **PIC was instructed to date mark all potentially hazardous food during time of inspection.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible in under-the-counter refrigeration units in food prep area.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food in food prep area and in walk-in cooler.
36 Outer opening observed around lower perimeter of door in rear of food prep area which does not protect against the entry of insects, rodents, and other animals.
36 The following were observed: -Visible physical evidence of rodent / insect activity (mouse feces, ants, dead water bugs) observed around perimeters of floor in food prep area dry storage area, and walk-in cooler -Dead water bugs observed on glue trap in rear food prep area **Dead or trapped birds, insects, rodents and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests.
41 The following were observed: -Knives stored between edges of cold prep refrigeration units in food prep area which are not easily cleaned and sanitized -Food dispensing utensil in the bulk ingredient containers in walk-in cooler stored in direct contact with food and not having handle stored above the top of the food -In-use utensils stored in cups of stagnated water at 73 degrees F **During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored in a container of water if the water is maintained at a temperature of at least 58°C (135°F) and the container is cleaned at frequency.
45 The following were observed: -Cardboard being used to line shelving units in food prep area/walk-in cooler which is not smooth and easily cleanable -Milk crates being used for elevation of food in walk-in cooler which is not smooth and easily cleanable -Soda crates being used for elevation of food in walk-in cooler which is not smooth and easily cleanable

52 Dumpster lids observed opened in outdoor dumpster area. **PIC took corrective action and closed dumpster lids during time of inspection.
53 Food debris/residue, static dust accumulation/debris, and mouse feces observed around perimeters of floor in dry storage area, food prep area, and walk-in cooler.
53 Brooms and dustpans observed stored on floor and not elevated on a utility rack designated for maintenance tools.
54 Hood filters have an accumulation of static dust and grease in food prep area.
