36 Visible physical evidence of rodent (mouse droppings) observed on the floor throughout the facility.

45 -The hand wash sink was not seal to the wall in the restroom area. -Defective under counter refrigerator observed in use for storage in food prep area. -Leaking hand wash sink faucet /ware washing area. -Missing a kick plate from the reach-in freezer/retail area.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. *The person in charge with a city of Philadelphia FSC showed up during inspection.
11 4 dented cans observed on shelf in retail area. * Dented cans were removed.
14 Black residue observed on the interior of ice machine dispenser of Big gulp soda machine.
36 Visible physical evidence insect activity (flies) observed in the food prep and ware wash area. ** Sanitarian advised the person in charge to hire a professional pest control service and provide receipt of service at next inspection.

13 Person in charge is not monitoring self service operation. Observed a customer removing cooked hot dog from the grill with his bare hands and another one holding half a gallon of milk to his nostrils to determine freshness by the hot beverage station.
