26 One spray bottles were found without a label to identify the name of the cleaner.

**The front door of the facility was observed open without a screen or other device to prevent the entry of pests.
**There is an open gap under the rear door that is not protected from pests. (door sweep)
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 Equipment Cleaning:
**Food residues and crumbs have accumulated on shelves inside the walk-in cooler.
**Food residues and crumbs have accumulated on surfaces inside the wok area refrigerator.
**There is a sticky residue on the handles to the wok area refrigerator.
53 Cleaning needed:
**Shelves near the front counter have a greasy residue accumulation.
**Cleaning and organizing is needed in the basement. Too much clutter. Any items not needed for the operation of the business should be removed.
53 Broom, dust pan, and/or mop were seen resting on the floor between uses rather than being stored up on a rack.
54 Lights in the kitchen are not shielded from breakage.
