Corrective action: Containers of beverages were moved above open container of egg batter for French toast.
-Uncovered sugar observed in the food prep area, and uncovered French toast batter observed in the reach-in refrigerator.
Corrective action: Sugar covered during the inspection.
14 Slicer has an accumulation of food residue on the food contact surfaces and is not being properly washed, rinsed and sanitized at least every 4 hours. After washing, accumulated food debris was observed on slicer.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 150 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions.
Corrective action: Concentration corrected.
20 Cheese stored above the cold line in the cold prep refrigerator and cheese and eggs in the food prep area held at 49.7 to 56.4 degrees F, rather than 45 degrees F or below as required.
Corrective action: Eggs and cheese at were discarded. Cheese stored above the cold line was placed in the lower compartment of the cold prep refrigerator.
Note: Items other than eggs, milk and cheese should be stored at 41 degrees F or below.
22 Time in lieu of temperature being used in the food facility to control potentially hazardous foods without written procedures or documentation to verify disposition of food. Eggs with a temperature of 56 degrees F were observed being placed in the reach-in refrigerator immediately after Sanitarian's arrival. Food employee stated that eggs were in the food prep area for less than an hour.
26 Observed multiple containers of insecticides not labeled by the manufacturer for food facility usage, in the food prep area on chemical storage shelf.
Corrective action: Items discarded.

-Uncovered container holding cut peppers observed nesting in uncovered onions.
Instructed food employee to cover all items in refrigeration units.
14 Black residue, pink slime observed on the interior of the ice machine bin at the top rear components and on wall. Food employee was instructed to turn the machine off, empty, wash, rinse, sanitize and air-dry the machine.
Corrective action: Emptying was initiated.
20 Potentially hazardous food was held at 45.6 to 51.3 degrees F rather than 41 degrees F or below as required.
-Eggs and cheese held at 49.8 to 50 degrees F, rather than 45 degrees F or below as required.
Corrective action: Tomatoes and eggs (above 50 degrees) were discarded and cold prep unit temperature was turned down, but within 45 minutes several items were still above 45 degrees F. Food was being moved to the walk-in cooler at the end of the inspection.

-Container lid and condiment squeeze bottle observed in the handwasing sink.
-Per owner, handwashing sink is used to washed food.
Corrective action: Items were removed from handsink and approximately 10 empty one-gallon single use drink jugs were removed from the food prep sink.
13 Heavy build-up observed on the waffle iron plates.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 100 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions.
Corrective action: Sanitizer concentration was corrected during the inspection, but the water was escaping through a faulty stopper.
20 Potentially hazardous food was held at 46.2 to 49.8 degrees F rather than 41 degrees F or below as required.
Corrective action: Person in charge was instructed to keep the door closed between orders; to move the cutting board that was keeping the door open; not to store food above the cold line; decrease the temperature of the unit; discard food above 45 degrees F. Some items were moved to colder units. Temperature in the bottom of the unit did not decrease. All food was moved to the walk-in cooler.
